Our staff is available by phone or in person 8 a.m. to 12 p.m. and 1 p.m. to 5 p.m., Monday through Friday.
Our office is located on the first floor of OSF Saint James - John W. Albrecht Medical Center. We are conveniently located off Interstate 55 at the Pontiac/IL-116 exit. Our address, phone number, fax number, and directions are listed below.

Scheduling
Appointments can be scheduled by calling our office at (815) 844-5343.
Some individuals have a difficult time scheduling doctor's appointments because of conflicts with their employment responsibilities. Our office will make every effort to accommodate your needs. Please let us know about any scheduling concerns you may have.
Cancellations: Should you need to cancel your appointment, we would appreciate at least 24 hours notice.

A visit to a surgeon's office can be stressful to many people. Don't be afraid to discuss your concerns with our staff. Feel free to write down a list to discuss with Dr. Proehl or Dr. Woodland.
For your convenience, some office procedures may be performed at the time of the initial evaluation.

Please allow time before your scheduled appointment to complete the questions on the following medical forms. Feel free to download and print these PDF files prior to your initial appointment.
Medications
It is essential that you provide an accurate list of your current medications including the dosage and frequency of drugs that you take as well as any allergies you may have. You may wish to bring your medications to our office for review. This is necessary to avoid reactions that could occur when anesthesia is being administered or during surgery.
Please Call Ahead
Emergencies are unpredictable but commonplace events in a surgeon's office. They require immediate attention and can sometimes delay your scheduled appointment.
For your convenience, please leave phone numbers (home, work, & mobile) where you can be reached so we can contact you in case of an unexpected waiting period. Your time is important and we welcome your calls to check for unexpected delays in our schedule.
